If you are interested in midwifery care, please fill out the intake form below. We will get back to you as soon as we can to let you know which midwife is able to care for you. Due to the high volume of interest in our care, we also keep a waiting list for those who we may not be able to accommodate right away.
Request Midwifery Care
Midwifery care does not require a referral in Ontario, and if you feel that midwifery is the right style of care for you, please fill out out online intake form, or contact either of our office locations for assistance. We will ask for a brief health or pregnancy history, as well as a best guess for what month you are due. We look forward to caring for as many families as we are able to, but we often hold a wait list. Please contact us as early as you are able in your pregnancy.
